In consideration of being allowed to participate in any league, tournament, event, clinic, or activity organized, operated, or sponsored by Match Point Sports Group LLC (“Match Point”), I acknowledge and agree to the following:

I understand that participation in pickleball and related athletic activities involves inherent risks, including but not limited to slips, falls, collisions, equipment failure, overuse injuries, and other physical or medical injuries. I knowingly and voluntarily assume all such risks, whether known or unknown, foreseeable or unforeseeable.

I hereby release, waive, and discharge Match Point, its owners, officers, employees, contractors, volunteers, affiliates, and agents from any and all claims, demands, actions, or causes of action arising out of or related to my participation.

I agree to indemnify and hold harmless Match Point from any loss, liability, damage, or cost, including reasonable attorney fees, arising from my participation or actions.

I affirm that I am physically able to participate and that I am solely responsible for my health and medical decisions. I understand that Match Point does not provide medical care.

I agree to abide by all Match Point rules, policies, and codes of conduct, including the Registration & Payment Policy, and understand that violations, eligibility determinations, or unsportsmanlike conduct may result in removal from participation without refund.

I grant Match Point permission to use photographs, video, or audio recordings of me for promotional, marketing, or informational purposes without compensation.

This waiver shall be governed by and interpreted in accordance with the laws of the state of Michigan.

I understand that by submitting this form electronically, I am providing a legally binding electronic signature.

Match Point reserves the right to request reasonable proof of legal consent for minor participants, and failure to provide such proof may result in removal from participation without refund.

I ACCEPT THE ABOVE AND AFFIRM THAT ALL INFORMATION I HAVE PROVIDED IS TRUE, ACCURATE, AND COMPLETE. I UNDERSTAND THAT PROVIDING FALSE OR MISLEADING INFORMATION MAY RESULT IN REMOVAL FROM PARTICIPATION WITHOUT REFUND.
