I assume all financial responsibility if my actions cause damage to the bar or significant damage to ChiTown Bagger's (Play Sports Chicago LLC) equipment.

I understand that if I need to be refunded for any reason I will not be reimbursed for the processing fee set by our payment processor.

I have read this release and waiver of liability, assumption of all risk, and indemnity agreement, fully understand its terms, understand that I have given up substantial rights by signing it, understand that I am assuming all risks, and have signed it freely and voluntarily without any inducement, assurance or guarantee being made to me and intend my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
