Registration Consent

By signing below, I hereby authorize Next Level Soccer Training to register myself, or my child or legal ward (as applicable), for participation in its programs, activities, training sessions, and events. I understand that registration with Next Level Soccer Training is exclusive, and that a player may not simultaneously participate with conflicting organizations without prior written consent.

Medical Treatment Authorization & Liability Waiver

I give my full consent for Next Level Soccer Training, including its coaches, trainers, team managers, staff, and any licensed medical personnel (such as EMTs, physicians, nurses, or dentists) and their affiliated personnel, to provide emergency medical care and treatment to the participant named above. I accept full financial responsibility for any such care or treatment.

I further authorize emergency transportation to a medical facility at my expense (or the expense of the parent/guardian, as applicable) if deemed necessary by any of the individuals above.

I understand that participation in soccer and athletic training carries inherent risks, including but not limited to sprains, concussions, fractures, and in rare cases, serious injury or death. These risks may arise from the actions of the participant, other players, staff, or unforeseen circumstances. I acknowledge and accept these risks.

I confirm that the participant is in good health and has received any necessary medical clearances to participate fully in all Next Level Soccer Training activities without restrictions.

To the fullest extent permitted by law, I voluntarily release, waive, and agree to hold harmless and indemnify Next Level Soccer Training, its owners, staff, contractors, volunteers, sponsors, and affiliated personnel from any and all claims, liabilities, or damages arising from or related to participation in training sessions, events, or transportation arranged by the organization.

Privacy Policy & Terms of Use

I acknowledge and agree that any personal or medical information provided to Next Level Soccer Training may be used solely for purposes related to program operations, medical treatment, safety, and communication. I understand that the organization will take reasonable measures to protect this information and will not share it without consent, except as required by law or for medical/emergency purposes.

Agreement

I certify that I have read and fully understand this agreement. I voluntarily accept and agree to all terms set forth in this Player Registration, Medical Authorization, and Liability Waiver on behalf of myself or the participant named above.

In consideration for the privilege to use the facility and/or attend the program, my signature indicates that I assume the risk of any injuries that I or my children may sustain while participating in any activity at Next Level Soccer Training, and for any injuries sustained while on its premises.

I confirm that I, or my child, am physically and mentally able to participate in physical activities and have been examined by a licensed medical physician within one (1) year prior to attending this program.

I hereby grant Next Level Soccer Training permission to use my likeness in photographs, video, or other digital media in all of its publications, including web-based publications, without payment or other consideration.

By signing this Waiver and Liability Agreement, I acknowledge that I have read, fully understand, and agree to all of its terms, including the Permission to Treat agreement. I further affirm that I have executed this waiver voluntarily and with full knowledge of its significance, and that it is binding upon me, my heirs, executors, administrators, and assigns.
